Intake Information
Please note that we do not accept walk-ins.
To schedule an intake please call 510-660-1493 and leave a voicemail with your name, phone number, any questions, and some good times to reach you.
During our initial call, we will answer any questions you may have, then schedule a day and time to conduct a 20-45 minute phone screening. During the phone screening we will tell you more about our clinic and get a better understanding about what brings you to therapy.
If it seems that we would be a good fit for your needs, we will send you a confidential email containing our electronic consent form, a link to complete online assessments, our intake questionnaire form, and an invoice for the intake appointment (The intake fee is $20 and can be paid by check or credit card. This fee is flat and non-negotiable).
Please read and sign the consent form, complete the online assessments, fill out the intake questionnaire form, and pay the $20 intake fee. Once we have received those items, we will schedule an intake appointment. Intakes consist of assessments and paperwork plus a 1.5-2 hour clinical interview with one of our clinicians.
